Assess the patient's ability to move their limbs on request.
Assess respiratory effort, depth, and the patient's ability to cough.
Compare current systolic blood pressure to the documented pre-operative baseline.
Assess the patient's level of alertness and responsiveness to verbal stimuli.
Modified version uses SpO2 from pulse oximetry; original version uses clinical skin colour.

  • A score of 9 or 10 meets the standard threshold for safe transfer from the PACU to a ward or discharge from an outpatient setting.
  • The Aldrete Score does not assess pain, nausea, vomiting, temperature, or surgical-site issues. Evaluate these separately before making a discharge decision.
