The R-IPI was developed for patients treated with R-CHOP and re-stratifies the original IPI into three groups with better prognostic separation. Use IPI for historical comparison.
Age above 60 at diagnosis is an adverse prognostic factor in both IPI and R-IPI.
Stage III = disease on both sides of the diaphragm; Stage IV = disseminated extranodal disease or bone marrow involvement.
Lactate dehydrogenase (LDH) above the upper limit of the local laboratory reference range is an adverse prognostic factor.
ECOG 0 = fully active; ECOG 1 = restricted but ambulatory; ECOG 2-4 = limited or bed-bound. Score 2-4 is adverse.
Extranodal sites include bone marrow, liver, lung, CNS, skin, gastrointestinal tract, and testes. More than one site is adverse.
