  • Begin at 300 mg on Day 1, increase to 600 mg on Day 2, and reach 900 mg/day (divided TID) from Day 3 onward.
  • For neuropathic pain / postherpetic neuralgia, the typical maintenance range is 900-1800 mg/day. The absolute maximum is 3600 mg/day.
  • Begin with 300 mg on Day 1, titrate up to an effective dose over 1-2 weeks. For postherpetic neuralgia the FDA label titrates to 1800 mg/day and states that additional benefit beyond 1800 mg/day was not demonstrated in the clinical studies; higher doses up to 3600 mg/day are used off-label for other neuropathic pain.
