Biological sex determines the IBW/PBW base constant in the Devine and ARDSNet formulas.
Patient height. The Devine formula requires a minimum of 60 inches (152 cm).
cm
ARDSNet lung-protective range is 4-6 mL/kg. Standard initial MV setting is 6-8 mL/kg. Values above 8 mL/kg are outside the protective range.
mL/kg IBW
Used to calculate target minute ventilation. Normal spontaneous RR is 12-20 breaths/min.

Ideal Body Weight (IBW)Lung-protective (ARDSNet)

Devine formula - the weight basis for ventilator tidal volume settings

TV - ARDSNet minimum (4 mL/kg)282mL
TV - target set volume423mL
TV - standard MV maximum (8 mL/kg)564mL
Target minute ventilation5.92L/min
Spontaneous breathing reference352-493 mL (5-7 mL/kg IBW reference)
ARDSNet min (4 mL/kg)282
Target volume423
Conventional max (8 mL/kg)564

IBW 70.5 kg - target tidal volume 423 mL at 6 mL/kg.

  • Devine IBW is 70.5 kg; the ARDSNet PBW formula gives 70.6 kg. Both are close and either is acceptable for setting ventilator volumes.
  • ARDSNet lung-protective range for this patient is 282 to 423 mL (4-6 mL/kg IBW). The conventional initial range is 282 to 564 mL (4-8 mL/kg IBW).
  • At 14 breaths/min, the target minute ventilation is 5.92 L/min. Normal adult resting minute ventilation is 5-8 L/min.

Next stepPlateau pressure should remain below 30 cmH2O with this volume. Monitor arterial blood gases and allow permissive hypercapnia if driving pressure is the limiting factor.
