What is beta-hCG and why does it matter?
Human chorionic gonadotropin (hCG) is a hormone produced by the developing placenta after a fertilized egg implants in the uterine lining. It is the hormone that home pregnancy tests detect. In early pregnancy, serial blood measurements of beta-hCG, the specific subunit tested in clinical labs, are one of the most important tools for assessing whether a pregnancy is developing normally. Levels can first be detected in blood around 10-11 days after conception and in urine around 12-14 days after conception. How to interpret doubling time
For a healthy intrauterine pregnancy when hCG is below 1,200 mIU/mL, doubling time is typically between 30 and 72 hours. Between 1,200 and 6,000 mIU/mL, 72 to 96 hours is considered normal. Above 6,000 mIU/mL, doubling slows naturally and times exceeding 96 hours can still be consistent with a healthy pregnancy. An increase of at least 35% over 48 hours is generally considered the minimum acceptable rise in early pregnancy. Levels that plateau, rise too slowly, or fall may indicate a failing pregnancy, miscarriage, or ectopic pregnancy, all of which require prompt clinical evaluation. What do very high or very low levels mean?
A single hCG result that falls outside the reference range for the week does not automatically indicate a problem, because the ranges span an enormous variation between healthy pregnancies. Very high hCG for the gestational week may point to a miscalculated due date, a twin or multiple pregnancy, or rarely a molar pregnancy. Levels that are low for the week may indicate a later-than-expected implantation, a pregnancy that is earlier than dated, a threatened miscarriage, a blighted ovum, or an ectopic pregnancy. Values between 6 and 24 mIU/mL are considered equivocal and usually require a repeat test within 48-72 hours. Only levels consistently below 5 mIU/mL are considered non-pregnant. A level of 1,000-2,000 mIU/mL is often used as a discriminatory threshold above which a gestational sac should normally be visible on transvaginal ultrasound.
hCG after pregnancy loss
After a miscarriage, surgical or medical termination of pregnancy, or successful treatment of an ectopic pregnancy, hCG levels decline. The rate of decline and the time to return to non-pregnant levels (below 5 mIU/mL) depend on the gestational age at the time of loss and the peak level reached. In most cases, hCG returns to non-pregnant levels within 4 to 6 weeks after a pregnancy loss in the first trimester. Monitoring the decline with serial measurements confirms that no pregnancy tissue remains. Levels that plateau during the decline may indicate retained products of conception or, in the case of ectopic pregnancy, persistent trophoblastic tissue requiring further treatment.
