What is mean sac diameter?
The gestational sac is the first structure visible on transvaginal ultrasound in early pregnancy, typically appearing as a small fluid-filled ring at around 4.5 to 5 weeks of gestation. Mean sac diameter (MSD) is calculated by measuring the sac in three perpendicular planes - its length, width, and height - and averaging those three numbers. The result is a single value in millimetres that represents the overall size of the sac. Because the gestational sac grows predictably at approximately 1 mm per day in early pregnancy, MSD can be used to estimate how far along the pregnancy is before a fetal pole is detectable.
How is gestational age estimated from MSD?
The formula is straightforward: gestational age in days equals MSD in millimetres plus 30. This relationship was described by Hellman and colleagues in 1969 and validated in subsequent studies. For example, a gestational sac measuring 18 mm in mean diameter corresponds to approximately 48 days (18 + 30) of pregnancy, or about 6 weeks and 6 days. The formula is most reliable between 4 and 10 weeks of gestation. After about 7 weeks, or once a fetal pole with measurable crown-rump length (CRL) is visible, CRL provides more accurate dating and should be used instead of MSD. The typical uncertainty of MSD-based dating is plus or minus 5 days.
What the ultrasound should show at each stage
Development inside the gestational sac follows a predictable sequence. A yolk sac - a small bright ring inside the gestational sac - normally becomes visible by 5.5 to 6 weeks (MSD around 10-17 mm). A fetal pole, the earliest form of the embryo, usually appears by 6.5 weeks (MSD around 15 mm), and cardiac activity is detectable by Doppler from about 6 to 6.5 weeks. If these landmarks are absent when the MSD suggests they should be present, a follow-up scan is generally recommended before drawing clinical conclusions, because a single measurement can be misleading if the dates are uncertain or if the sac was measured slightly off. An empty gestational sac with MSD above about 25 mm and no yolk sac, or above about 25 mm with no fetal pole, may be evaluated with the appropriate clinical criteria.
Limitations and when to use CRL instead
MSD has several practical limitations. First, sac shape is irregular, so even careful three-plane measurement carries operator variability of a few millimetres. Second, the MSD + 30 formula assumes average growth; early implantation of abnormal pregnancies may show a smaller-than-expected sac. Third, once a fetal pole is visible, crown-rump length becomes the gold standard for first-trimester dating and should replace MSD. The American Institute of Ultrasound in Medicine (AIUM) practice parameters recommend that gestational age assigned at the first ultrasound should not be revised by a later scan unless the discrepancy exceeds a defined threshold, so the earliest reliable measurement carries the most weight for setting the estimated due date.

What is a normal mean sac diameter for 6 weeks?
Rearranging the dating formula gives MSD (mm) = gestational age in days minus 30. Six weeks equals 42 days, so the expected mean sac diameter at 6 weeks is about 12 mm. Allowing for the plus or minus 5 day accuracy band, an MSD anywhere from roughly 7 to 17 mm is consistent with 6 weeks. A single measurement is always interpreted alongside clinical context.
How accurate is MSD for dating pregnancy?
MSD-based dating has a typical accuracy of about plus or minus 5 days. It is most useful when the last menstrual period is uncertain, or when MSD and LMP-based dates agree within that margin. Why do some reports show MSD in centimetres instead of millimetres?
Both units appear in clinical practice. Many ultrasound machines default to centimetres for larger measurements and millimetres for smaller ones, so a report might list dimensions as 2.0 cm, 1.8 cm, 1.6 cm.
