How Clomid and Letrozole trigger ovulation

Clomid (clomiphene citrate) and Letrozole (Femara) are the two most commonly prescribed oral ovulation induction medications. Clomid works by blocking estrogen receptors in the hypothalamus, tricking the brain into producing more follicle-stimulating hormone (FSH) and luteinizing hormone (LH), which stimulate the ovaries to develop follicles. Letrozole is an aromatase inhibitor: it reduces estrogen production, which raises FSH levels in a more targeted way, often producing fewer mature follicles and a lower multiple-pregnancy risk than Clomid. Both medications are typically taken for 3 to 7 days early in the menstrual cycle (most often days 3 to 5, or days 5 to 9), and ovulation follows once the medication course ends and FSH rises further.

When to expect ovulation after your last pill

After taking the final tablet, the body typically mounts an LH surge within 5 to 10 days for Clomid (median about 7 days) and 5 to 12 days for Letrozole (median about 8 days). The wide range exists because follicle growth rate varies between individuals and cycles. Ovulation occurs roughly 36 hours after the LH surge peaks. Ovulation predictor kits (OPKs) can detect the surge 1 to 2 days before egg release; start testing about 3 to 4 days before your calculated ovulation date. The fertile window spans 5 to 6 days: the 5 days before ovulation and the day of ovulation itself, because sperm can survive in the female reproductive tract for up to 5 days while the egg is viable for only 12 to 24 hours after release. Implantation and the two-week wait

Implantation is when the embryo begins producing human chorionic gonadotropin (hCG), the hormone detected by home pregnancy tests. hCG doubles roughly every 48 to 72 hours in early pregnancy, so testing too early may give a false negative. Waiting 14 days after expected ovulation before testing gives enough time for hCG to accumulate to reliably detectable levels on a standard home test. Most guidelines recommend no more than 3 to 6 consecutive Clomid cycles before reassessing and potentially moving to injectable gonadotropins or other assisted reproductive technologies. Letrozole shows comparable or slightly better outcomes in some studies, particularly for women with polycystic ovary syndrome (PCOS).
