Worked example

A man has testosterone of 550 ng/dL and estradiol of 25 pg/mL. T/E2 = 550 / 25 = 22.0. Both values are within the normal male ranges, and the ratio of 22 falls within the research-reported optimal band of 10-30. If the same testosterone were reported as 19.1 nmol/L, it would first be converted: 19.1 x 28.842 = 550 ng/dL before dividing.

What is the testosterone to estradiol ratio?

The T/E2 ratio is simply your total testosterone concentration divided by your estradiol (E2) concentration, both expressed in the same unit system. Because testosterone and estradiol are reported in very different units by most labs (ng/dL versus pg/mL), the conventional approach is to divide testosterone in ng/dL by estradiol in pg/mL without further unit conversion. This produces a dimensionless number that researchers have linked to several aspects of male hormonal health. A typical value in healthy adult men falls between 10 and 30 by this convention, though there is no single universally agreed cutoff that defines "healthy." What matters most is that each hormone sits within its own normal range. The ratio adds a layer of context: two men with the same testosterone level can have very different hormonal environments depending on how much of it has been converted to estradiol.

Why does the T/E2 ratio matter clinically?

Testosterone and estradiol are interconnected through the enzyme aromatase, which converts testosterone into estradiol in fat tissue, the liver, the brain and other organs. Factors that affect the T/E2 ratio
