How tramadol works

Tramadol is a centrally acting analgesic that achieves pain relief through two separate mechanisms. First, it binds to mu-opioid receptors in the brain and spinal cord. Second, it inhibits the reuptake of serotonin and norepinephrine, which modulates pain signals descending from the brain. Immediate-release vs extended-release: choosing the right formulation

Immediate-release (IR) tramadol typically comes in 50 mg tablets taken every four to six hours as needed. It is the standard choice for acute or intermittent pain and allows flexible dosing. The extended-release (ER) formulation is taken once daily and is designed for patients who need continuous, around-the-clock analgesia for chronic pain. ER tablets must never be crushed, split, or chewed because releasing the full dose at once can be dangerous. ER is not appropriate for patients with a creatinine clearance below 30 mL/min or for those with severe hepatic impairment, conditions that the IR formulation can still be used in, albeit with a reduced dose.

Special populations and dose adjustments

Kidney impairment slows tramadol clearance so that the drug accumulates to higher-than-expected levels. When creatinine clearance falls below 30 mL/min, the prescribing label specifies extending the dosing interval to every 12 hours and capping the total daily dose at 200 mg with the IR formulation.
